Championship Information Request

In an effort to streamline the WEGA Championship Entry process, we are looking to change our method of collecting applicant entry information and Championship Entry Registration to a primarily electronic/digital process.  To help us in this endeavor, we are asking anyone interested in participating in any of our WEGA Championships to provide us with the following information so that we can set you up in our database.
