1. one for $90.00 payable to WEGA
2. one for $110.00 * payable to Shackamaxon Golf and Country Club
  
  * if a Shackamaxon caddie is employed for a minimum of three
  days (paying caddie directly) the Host Club Fee is $60.00 (instead of $110.00) payable to Shackamaxon Golf and Country Club. You must indicate on the entry form that you intend to take a Shackamaxon caddie.
